Polara Health is seeking a dynamic and compassionate Inpatient Family Nurse Practitioner (FNP) to join our healthcare team. ESSENTIAL RESPONSIBILITIES
· Provide comprehensive primary care services, including physical exams, diagnosis, and treatment of acute and chronic conditions on our inpatient units.
· Conduct comprehensive assessments and develop an individualized medical treatment plan.
· Order and interpret diagnostic tests and develop treatment plans.
· Prescribe medications and other therapies.
· Communicate effectively with patients, families, and other healthcare professionals.
· Maintain compliant, accurate, and up-to-date patient medical records, documentation, and billing practices.
· Stay current with advancements in primary care hospital medicine and nursing practices.
QUALIFICATIONS
· 21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member’s scope of practice; or minimum 18 years old for select administrative positions
· Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
· Must possess and maintain a valid Driver License, current automobile insurance, and a driving record that meets the guidelines and requirements of the organization
CPI and CPR/First Aid certification required (training provided by Polara Health)
Master's or Doctoral degree in Nursing (MSN or DNP) and certification as a Family Nurse Practitioner (FNP)
Board certification from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP)
State of Arizona active nursing license in good standing
One or more years’ experience working as an inpatient nurse practitioner
Experience in primary care or family practice settings
Strong clinical skills in diagnosing and treating a wide range of health conditions
Excellent communication and interpersonal skills
Proficiency with electronic health records (EHR) systems
Ability to work independently and as part of a multidisciplinary team
Commitment to preventive care and health education upon patients’ discharge
Strong organizational and time management skills
Strong clinical skills and a patient-centered approach to care

PHYSICAL DEMANDS & WORKING CONDITIONS
The nature of work may generate considerable physical (transporting, moving, assisting clients, etc.), mental and visual demands. · Office environment, travel within the community, as needed
· Heavy exposure/use of computers, cell phones, office phones, etc.
· May be required to wear protective clothing (PPE)
· Annual TB screening per program policy and procedure
· Ability to be physically mobile (stand, walk, reach, climb, bend) and the ability to sit or stand for prolonged periods throughout work hours
· Must be able to occasionally lift and/or move 50 or more pounds
· Must be able to respond appropriately and effectively in crisis situations by using good judgment and following Polara Health protocols
· Must be able to successfully complete all physical skills components of CPI, including floor-based interventions
