POSITION SUMMARY

We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our team. The ideal candidate will manage a diverse patient panel, including acute, chronic, and preventive care, in an integrated primary care setting. This role involves collaboration with behavioral health providers, specialists, and support staff to ensure holistic, person-centered care.

ESSENTIAL RESPONSIBILITIES

Manage a patient panel with a broad array of needs, including acute issues, chronic diseases, and wellness promotion.
Provide high-quality medical care to adult and/or family populations.
Utilize the Electronic Health Record (EHR) to document and coordinate patient care effectively.
Collaborate with behavioral health providers, specialists, and external agencies to ensure integrated care.
Support a team-based care model by mentoring staff and guiding coordinated care efforts.
Participate in clinical team meetings and interdisciplinary collaboration efforts.
Assist with onboarding and mentoring new providers and staff members.
Review and apply data on patient outcomes, patient satisfaction, and operational efficiency to guide care decisions.
Maintain Polara Health Medical Staff membership and compliance with credentialing requirements.

QUALIFICATIONS
· Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
Must possess a valid Driver License
CPI and CPR/First Aid certification required (training provided by Polara Health)
Licensed as a Family Nurse Practitioner or Physician Assistant with advanced practice certification in Family Practice in the State of Arizona.
Current Arizona Prescribing and Dispensing Authority and DEA certification.
· Eligible for Polara Health Medical Staff membership
Preferred: Two years of experience in a primary care or family practice setting.
Experience in an integrated care or behavioral health setting is a plus.

PHYSICAL DEMANDS & WORKING CONDITIONS
